Dentists

Application Types


Fees are non-refundable and NO CASH WILL BE ACCEPTED.
  • General Dentist Application by Exam - This is the application method for Dentists that have taken a Regional Exam within the last 5 years.

    The following exam components are required for a Dentist:
    • Prosthodontics
    • Periodontics
    • Endodontics
    • Anterior Class III and Posterior Class II restorative on a live patient or manikin 
    • Diagnosis and Treatment Planning Section
  • General Dentist Application by Credential - This is the application method for Dentists who have taken a Regional Exam longer than 5 years ago, who are licensed in another state and have been practicing 5 years immediately prior to turning in the application.

Please read all documents and checklists BEFORE calling the Board office.

Fees are listed at the top of the application and are NON-REFUNDABLE.

  1. All applications for dentist licenses must be approved by the Board.  Applications must be received 30 days prior to a meeting in order to be placed on the next regularly scheduled Board agenda. For Board meeting dates, please refer to the Reports/Agendas tab of our website.
  2. After your application has been approved by the Board, you will be contacted via email to schedule a date/time to take the Jurisprudence Exam.  You may begin studying for the Jurisprudence Exam immediately by clicking on the following link Statutes & Rules
  3. Exams are only given on Thursdays by appointment only and are only proctored at the Board office.
