Application Types

Just a note- all added advanced procedures are a ONE time fee, please do not add an additional $10 to your check or money order when you are renewing by paper form and already have the advanced procedure on your license.  Also, new applicants: do not add $10 for Local and Nitrous, only do so if adding after initial licensure.  All fees are non-refundable and WE WILL NOT ACCEPT CASH.


  • Hygiene Application by Exam- This is the application method for Dental Hygienists that have taken a Regional Exam within the last 5 years. 

    The following exam components are required for a Hygienist:
    • Clinical patient treatment with an evaluation of specific clinical skills as well as the candidate's compliance with professional standards during the treatment and shall include:
      • Extra/Intra Oral Assessment
      • Periodontal probing
      • Scaling/subgingival calculus removal and supragingival deposit removal

 

  • Hygiene Application by Credential - This is the application method for Hygienists who have taken a Regional Exam more than 5 years ago, who are licensed in another state and have been practicing 5 years immediately prior to making application.

(6) Vaccinations, venipuncture and phlebotomy

(A) Definition: A dental hygienist may administer vaccinations or draw blood for a platelet rich plasma procedure (PRP) or other testing under the direct or indirect supervision of a dentist.\

(B) Requirements: Completion of a phlebotomy certification program as approved by the Board. A course approved by the Board shall include a minimum of four (4) hours of safe-needle and blood-born pathogens procedures under the guidelines of the CDC.
