NOTICE

****ALL DENTAL ASSISTANTS MUST HAVE A PERMIT BEFORE WORKING CHAIRSIDE AS OF NOVEMBER 1, 2025.****

 

** Please note board staff is about a week to 10 days behind on renewals, there are only two people processing renewals and we are doing our best to get caught up.  PLEASE DO NOT CALL THE BOARD OFFICE UNTIL YOU HAVE VERIFIED YOU ARE NOT UPDATED ON THE WEBSITE FIRST (You will need to give at least 10 days from the date you submit your renewal before contacting us.) ** (Dentist and Hygienists are caught up please contact Ashley Adkins if you have not received your license)

Dental Assistants

Fees are non-refundable and NO CASH WILL BE ACCEPTED.

Each application MUST have a color copy of your driver's license or photo ID submitted.

  • New Dental Assistant Application - This is only for Dental Assistants that have never registered in Oklahoma before.  You must obtain a Dental Assistant Permit before applying for any expanded functions. Please DO NOT send cash with application, must be check or money order only.   For additional questions please email Naukeshia Johnson.  THIS IS NOT A RENEWAL APPLICATION!!!!
  • Each Credential Application MUST have the out of state form completed and supporting documentation turned for the application to be complete.
  • Dental Assistant Application by Credential-This is only for Dental Assistants coming from out of state who have already obtained expanded functions and are requesting to add the duties when applying for a Dental Assistant permit.
  • Expanded Function Application - for X-Ray, Nitrous, Sealants, Coronal Polishing, Elder Care/Public Health, Venipuncture, Phlebotomy
  • Oral Surgery Expanded Function Application- For Oral Surgery Assistants only.

Vaccinations, Venipuncture, and Phlebotomy Courses

Requirements:

(6) Vaccinations, venipuncture and phlebotomy.

(A) Definition: A dental assistant may administer vaccinations or draw blood for a platelet rich plasma procedure (PRP) or other testing under the direct supervision of a dentist. 

(B) Requirements: Completion of a phlebotomy certification program as approved by the Board. A course approved by the Board shall include a minimum of four (4) hours of safe-needle and blood-born pathogens procedures under the guidelines of the CDC.
