Step 1 — Apply Online

Step 2 — Pay Online

Step 3 — Watch for AdobeSign
If you already applied online, you will receive an AdobeSign email with your completed application.
Step 4 — Email Us Your Completed Packet
Please email wrestling@osac.ok.gov, along with:
- A screenshot of your payment confirmation
- Hepatitis B Surface Antigen bloodwork results
- Hepatitis C Antibody bloodwork results
- HIV bloodwork results
- Physical examination signed by an MD/DO within 365 days
Step 5 — If You Fax Medicals
If your medicals are faxed to 405-900-8383, please let us know when they were sent or when we should expect them, so we can ensure they are properly matched to your application.
Step 6 — Processing Time
Once everything is received, processing typically takes 24–48 business hours (3–5 days). You are responsible for sending everything in so processing is timely.
Please note that applications are not processed on Friday-Sunday.
