Combative Sports Practitioners Apply Online
Combative Sports in Oklahoma
- License Validity: Licenses are valid from July 1 to June 30 each year.
- Sports: You need a separate license for each sport you participate in.
- Find Your Sport: Start your online application by selecting your sports application.
- National or Federal ID: If you are debuting or renewing or replacing your National or Federal ID, you’ll need to fill out that application too.
- Once you submit your application you will need to email boxing@Health.ok.gov and submit 2 forms of Government ID's, a current photo and arrange payment.

Bloodwork Required
- Blood work is valid for 365 from the collection date
- Hepatitis B surface Antigen
- Hepatitis C Antibody
- HIV
- All bloodwork must be official result with Full Name, Date of Birth, lab information, and collection date.
