- Fill out your application and submit.
- Submit your Physical & Bloodwork by email, mail or fax.
- Finalize with payment.
- NOTE: July 1st 2025 - No second or referee license will be issued you must have a Professional Wresling License.

- License Validity: Licenses are valid from July 1 to June 30 each year.
- Sports: You need a separate license for each sport you participate in.
- Find Your Sport Below: Start your online Oklahoma License application by selecting your sports application.
- National or Federal ID: If you are debuting or renewing or replacing your National or Federal ID, you’ll need to fill out that application too.
- Once you submit your application you will need to call (405) 426-8035 to arrange your payment.
- Payment and Applications are processed Monday - Thursday 10am-4pm.
