HealthChoice Medicare Supplement Plans Handbook

This Medicare supplement handbook/Evidence of Coverage replaces and supersedes any Medicare supplement handbook/Evidence of Coverage the Office of Management and Enterprise Services Employees Group Insurance Division previously issued. This Medicare supplement handbook/Evidence of Coverage will, in turn, be superseded by any subsequent Medicare supplement handbook/Evidence of Coverage EGID issues.

This handbook, your enrollment form, confirmation statement and HealthChoice SilverScript Medicare documents represent our responsibilities to you. This handbook provides details about your benefits, formulary, pharmacy network, premiums, deductibles, copays and coinsurance for 2023. It explains what is covered and what you pay as a member of the plan. Be aware that these amounts may change at the beginning of the next plan year, which begins on Jan. 1.
