Use the Portal to apply or renew.

Many of our forms are available on our online license portal. However, there are a number of forms that must be still be filled out manually and uploaded into the system. Those forms can be found in the relevant sections below.

Apprentice Employment Affidavit

This form is for two purposes:

  • To document employment with your original apprentice application.
  • Those currently registered apprentices who have changed their funeral home employment since becoming registered or need to add, change, or remove a funeral director or embalmer who will be supervising their apprenticeship.

Qualifications, reporting requirements, and rules regarding apprenticeships may be found in 235: 10-3-5 and §59-396.11 of the Oklahoma Funeral Board rules and statutes.

Oklahoma State Law Exam Information

This form explains what steps you will need to take to apply to take your law exam and what to expect. Click on the Rules and Statutes tab towards the top to study in order to pass the law exam.

Retake Oklahoma State Law Exam

Should you not receive a passing grade on the Oklahoma State Law Examination for Funeral Directors, you will need to complete the retake section using the portal.

Reciprocal Funeral Director or Embalmer

Fill out this form if you are licensed as a funeral director or embalmer in another state and have intention of practicing in Oklahoma. Should you have any questions regarding qualifications and requirements or licensing procedures, please reference §59-396.8 of the Oklahoma Funeral Board statutes.

Lapsed License Renewal

If you are wishing to renew your license after being inactive, complete the renewal in the application portal.

Duplicate License Request

Visit the portal and complete a document request for duplicate license if you are needing a duplicate wall license. Please allow at least four weeks for processing.

Funeral Establishment Forms

This form is if you are applying for either a new funeral home establishment in Oklahoma or if you are transferring ownership of an existing funeral establishment in Oklahoma. Reference 235: 10-3-2 of the Oklahoma Funeral Board rules regarding qualifications and requirements. If you are the owner, please fill out this form.

Change of Funeral Director in Charge Guide

This document explains exactly how to go about changing the Funeral Director in Charge (FDIC) for a currently licensed funeral establishment, commercial embalming establishment, or crematory. Per title 235:10-5-1 (a)(10) of the Oklahoma Funeral Board rules and statutes, the fee for a change of Funeral Director In Charge is $500.00 for each establishment.

Change of Establishment Name Guide

This document explains exactly how to go about changing strictly the establishment name for a currently licensed funeral establishment, commercial embalming establishment, or crematory. If there is a change in ownership, please use the corresponding application above. Per title 235:10-5-1 (a)(15) of the Oklahoma Funeral Board rules and statutes, the fee for a change of establishment or crematory name is $250.00.
