Identity theft happens when someone steals your personal information and uses it without your permission with fraudulent intent, including to obtain, or attempt to obtain, credit, goods, services, any other thing of value, or medical
information in the name of another person without the consent of that person.
What should you do if you think someone has filed a
fraudulent return using your name or Social Security number?
1. File with the IRS the IRS Affidavit form 14039. Send a copy of IRS Affidavit form 14039 along with supporting documentation to the Oklahoma Tax Commission.
2. Provide a signed statement, including a daytime contact phone number, that you are a victim of ID theft or fraudulent return filing.
To combat identity tax fraud, the OTC is requesting additional information from
taxpayers to ensure your tax refund is received by you.
• When e-filing, you may be asked for more information.
• You may receive an identity verification letter from the OTC after filing to verify your identity to
receive your refund.
• If filing a paper return, indicate on the bottom of the page one of the 511 or 511NR “Identity Theft”
