This application is filled out for manufactured items exceeding 16ft in width.
Prior to ordering Oklahoma Size & Weight permits, a company profile needs to be filled out along with a copy of the person’s driver’s license submitting the application so that company information can be placed into our program accurately.
This form is for loads that are 15’9” or greater in height. Also, a link for Union Pacific Railroad needs to be added to the 4th page of the Contact List for Utilities and Railroads: https://www.up.com/real_estate/house_moves/index.htm
