License Verification

If you are a licensee that is needing to request a license verification, you will need to submit the form below along with your payment to the address on the form. Once your request and payment are received, OBESPA will process your request within 3-5 business days.

License Verification Request Form

• Cost for each verification is $5.00, we accept check or money order made payable to OBESPA
• If a state has provided their own verification form for OBESPA to fill out, you will only need to provide their form and the payment
• If the entity/agency accepts license verifications electronically, OBESPA will need the email address AND mailing address
• This form will not be accepted by email

To search for an active licensee please click the link below. If the licensee you are searching for does not appear they either do not hold a current Oklahoma license OR they are in the application process & are waiting Board approval.

• When searching for a licensee, you can only enter one or two searching criteria (Example: Last Name & License Type OR Last Name & License Number) or the system will give you an error.
• Complaints or actions against a licensee will not appear online. If you are needing that information please email Mary.Johnson@obespa.ok.gov.
