Licensure for SLPA

  1. Completed Application
  2. Passport Photo
  3. Application Fee of $85.00
  4. Citizenship Affidavit
  5. Supervisor Letters of Agreement
  6. Official Transcripts
  7. Background Check from Trak-1
  8. Supervisors 6 hours of CEU Training

Other Possible Forms Depending on Qualifications:

  1. Clinical & Academic Verification Form
  2. Copy of Current ASHA SLPA Certification Card or ASHA SLPA Certification Letter   OR
  3. License Verification Letter, Copy of Licensure Card & Copy of Rules from Another State.
