Inspections

No other individual or company is allowed to conduct inspections and/or issue a Certificate of Occupancy for, or on behalf of the Oklahoma State Fire Marshal’s Office.

All permitted projects are required to be inspected for compliance with the permit and the applicable codes. Please read the instructions in the building permit comments issued for your project to find out what needs to be done before inspections can be requested, including what additional permits are required, if any. 

Inspections are performed by OKSFM Agents.

Inspection requests must allow at least 7 days of advance notice. 

50% inspections will not occur until all required permits are issued and on site. 

One (1) 50% inspection is provided at no cost. 

Inspections will be coordinated with all other permitted trades so that only 1 (one) site visit is necessary. If there are multiple permits involved with the project, a general contractor or owner is required to request the inspections. Inspection requests will not be accepted by subcontractors if there are multiple permits involved. 

50% inspection requirements:

  • All color permits, permit comments, and approved / full color stamped plans
  • Building weather-tight
  • Sheet rock on one side of all walls, but no door jambs. At least one side of walls exposed.
  • No ceilings
  • Label rated walls by permanent stencil
  • Fire extinguisher cut out provided 
  • Fire Doors, ratings, and UL Design (Provide copy of UL documents)
  • Fire Windows, ratings, and UL Design (Provide copy of UL documents)
  • Fire sprinkler rough in (if applicable)
  • Fire alarm rough in (if applicable)

Agents are permitted to alter the 1 (one) 50% visit when they deem it necessary. Agents may elect to perform an 80% inspection at their discretion. 

Contact the Oklahoma State Fire Marshal's Office at (405) 522-5005 to schedule a 50% completion inspection. You will be asked to provide the building permit number for each permit you are wanting inspected, a point of contact name, phone number, and email address.

If there are any changes to the project after the plans have been approved and permit issued, an addendum will be required to be submitted, reviewed, and issued before final inspections can occur. Although the project is reviewed and permitted, it is not uncommon for Agents to identify issues that were unforeseen during the design and review processes. Agent may issue order to be completed before the 100% inspection

100% inspection requirements:

  • All color permits, permit comments, and approved / full color stamped plans
  • Any instructions from previous inspection reports from the agent completed
  • All permits and addendums, if applicable, are required to be issued before the 100% inspection
  • All permitted aspects of the project must be completed
  • Certificate of Completion – Final Statement of Compliance Form that is available on this page or from an OKSFM Agent. The form may be legibly filled in by hand or via the fillable form for printing and final signatures. 
    • This form is now used in lieu of previous final contractor statement of compliance letters (foundation, structural, and MEP) unless otherwise noted by the OKSFM and agents performing inspections.
    • If the project was designed and plans sealed by a registered design professional such as an Architect or Engineer, then the ARCHITECT and or ENGINEER will be responsible for completing the form and obtaining the required signatures.
    • If the project was not designed or sealed by a registered design professional, then the OWNER is responsible for completing the form and obtaining the required signatures from each contractor.
    • NOTE: The Certificate of Completion-Final Statement of Compliance Form is NOT a Certificate of Occupancy
  • There are tests which occur for systems (sprinkler, fire alarm, carbon monoxide, carbon dioxide, smoke control, access control, etc.) It is vitally important that all contractor tests be successfully completed prior to the 100% OKSFM inspection and documentation provided at the inspection.
    • Fire Alarm: Monitoring Agreement and Record of Completion/Acceptance Test
    • Fire Sprinkler: Underground Flush, Record of Completion, Hydrostatic Test Certificates, FDC/Fire Chief Letter 
    • Kitchen Hood Suppression: Hood Suppression Acceptance Test Certificate 
    • Clean Agents, etc.: Alternative Suppression Record of Completion/Acceptance Test
    • Access Control: Record of Completion Letter from installing company
    • Carbon Dioxide (CO2): Record of Completion Letter from installing company
    • Smoke Control: Record of Completion/Engineers Acceptance Test Report

Inspections will be coordinated with all other permitted trades so that only 1 (one) site visit is necessary. If there are multiple permits involved with the project, a general contractor or owner is required to request the inspections. Inspection requests will not be accepted by subcontractors if there are multiple permits involved. Agents may elect to perform an 80% inspection at their discretion.

Use of the 100% inspection to create “punch lists” will create additional costs and time delay. If the facility did not follow the approved plans, the inspection will not be approved until an addendum is applied for and issued.

One (1) 100% inspection is provided at no cost. Projects which fail any 100% Final Inspection shall be subject to re-inspection fees of $100 per inspection site visit. 

Once the project has passed the final inspection by an OKSFM Agent, the Agent will recommend a Certificate of Occupancy be granted. The Certificate of Occupancy will be emailed by the OKSFM office to the point of contact listed for the project or inspection.

Projects which fail any 100% final inspection shall be subject to re-inspection fees of $100 per inspection site visit.

Other types of inspections can be requested if the OKSFM is the Authority Having Jurisdiction for your facility. Please view the AHJ list linked on the home page of our website. If you are located within the limits of an entity on the list, please reach out to that entity for your inspection requests.
