All personally identifiable data is stored on secure OMES servers, maintained by OMES. (OMES- State of Oklahoma Office of Management Enterprise Systems)

This data is shared with the Oklahoma Tax Commission and OPhA, and upon written request, OSBP will share employment location history of Registered Pharmacists and Pharmacy Technicians.

The Oklahoma State Board of Pharmacy collects and stores the following personally identifiable data:

• Place of Birth
• Mother's maiden name
• Biometric Records
• Social Security Number
• Official State or Government-Issued Driver's License
• Alien Registration Number
• Government Passport Number
• Employer or taxpayer identification number
