The Oklahoma State Board of Pharmacy certifies that it maintains the information for the license verification function of this website, performs daily updates to the website and considers the website to be a secure, primary source for license verification.
Written Verification of Licensure (Letter of Good Standing)
There is a $10 fee for a certified verification of licensure from the Board. Website verification remains free.
Interns
Do you need a renewal application paper form, specialized permits, duplicate certificate, or licensure information?
Announcements
Updates to registrant records such as expiration dates, name changes, and additional permits (i.e. preceptor, immunization) are updated on the Board's license verification page nightly after 7 pm.
Please remember to allow at least two weeks for paper applications to be processed. Online renewals should allow 24 hrs for the new information to appear.
