Residential application:
You will contact DDS.RESIDENTIAL@okdhs.org. Then you will be invited to a potential new provider class. Only after attending the entire class will, you be given a new provider application.
Vocational application:
You will contact DDS.Vocational@okdhs.org. Then you will be invited to a potential new provider class. Only after attending the entire class will, you be given a new provider application.
Evidence of sufficient liquid assets or lines of credit in the name of the provider agency or business indicating the greater of $100,000 or three times the average monthly-budgeted expenses.
The line-item budget should reflect what services a provider expects to provide within the first year of operations.
- $250,000 professional liability minimum, when the provider agency receives $50,000 or less per fiscal year for the delivery of supports funded through OKDHS or Home and Community Based Services Waivers (HCBS) or $1 million when the provider agency receives more than $50,000.
- $100,000 general liability minimum, when the provider agency receives $50,000 or less per fiscal year for the delivery of supports funded through DHS or HCBS Waivers or $1million when the provider agency receives more than $50,000.
- $100,000 commercial automobile liability minimum per occurrence.
- $25,000 minimum employee dishonesty coverage.
Upon coverage commencement, a copy of the certificate of insurance with a 30-calendar day cancellation notice is required and, is sent to DDS by the carrier showing OKDHS as the certificate of insurance holder.
Currently, the group home services are not authorized for new providers.
Residential:
The executive director or program director has a bachelor's degree from an accredited college or university and a minimum of two-years supervisory or management experience.
The program coordinator has a minimum of four years of any combination of college level education or full-time equivalent experience in serving persons with disabilities, or full-time equivalent experience in a supervisory position, unless the DDS director or designee waives this requirement.
Vocational:
Program Manager – must have a minimum of 4 years of qualifying experience of any combination of education or full-time experience with individuals with disabilities.
Job Coach – must have a minimum of a high school diploma or a GED and certification through DDS approved training courses.
Prior to submission of this application, grievance policies need to be first approved by the Office of Client Advocacy (OCA) oca.grievances@health.ok.gov.
You must attach a copy of the grievance policy and approval letter to the application.
You must identify your internal grievance process and grievance coordinator in the application. The identified grievance coordinator must complete local grievance coordinator training which is provided by the Office of Client Advocacy.

A Group Home offers living arrangements for 6 to 12 people who share a home and receive up to 24 hours per day of supervision, support, and training in daily living skills. Group Home residents are 18 years of age or older. Group Homes are single-family homes located in the community close to other services and activities.
A Daily Living Supports (DLS) home is a program designed to support adults to be as independent as possible in daily living and community integration. Individuals approved for DLS typically live with two housemates and have an agency responsible to meet their daily care, medical needs, and provide opportunities for community integration.
Yes, any setting which segregates individuals from the community must ensure ‘regular and frequent opportunities to participate in community-based activities which support the service they are enrolled in.
