Application for Appointment in Non-Capital Trial Cases
Non-Capital Trial Conflict Counsel Representation - To be approved to provide Conflict representation for the Oklahoma Indigent Defense System in Non-Capital Trial matters, this form must be submitted along with a copy of a letter of Good Standing from the Oklahoma Bar Association and a copy of your Declaration of Liability Insurance. Information on this form will be used to make assignments on a case-by-case basis.
Non-Capital Trial Participating Counsel Application - To be approved as a participating attorney on an existing NCT county contract representing clients in Non-Capital Trial matters, this form must be submitted along with proof of Good Standing from the Oklahoma Bar Association, a copy of your Declaration of Liability Insurance and a letter from the contracting attorney requesting your addition to his/her contract

Application for Appointment in Appellate Cases
Appellate Counsel Application - for private attorneys willing to accept assignments of OIDS cases on appeal. Information on this form will be used to make assignments on a case-by-case basis as the need arises in accordance with the experience level and interests of the private attorney.
Application for Investigators
Investigator Application - In order to be approved for providing investigative services to the Oklahoma Indigent Defense System, this form must be submitted along with current legible copies of professional licenses. In accordance with state statute, all investigators are paid an hourly salary of $30 per hour.
Application for Appointment in Capital Trial Cases
Capital Trial Application designed to facilitate the determination of whether an attorney possesses the minimum necessary qualifications under these Guidelines to be included on the rosters of attorneys who are willing to accept appointments as either Lead Counsel or Co-Counsel to represent an indigent defendant at the trial level in capital murder cases.
Professional Service Provider Forms
In order to provide expert services to the Oklahoma Indigent Defense System, an Expert Application must be submitted to the agency and a Supplier ID must be created through the Office of Management and Enterprise Services (OMES).
The Expert Application & Fee Schedule must be submitted to the Oklahoma Indigent Defense System along with a Curriculum Vitae or Resume' and a legible copy of any professional license the applicant may hold.
