How do I apply for a License of State Authorization as a seminar?
- Contact the OBPVS Staff for the main application and instructions.
- Download the remainder of the Forms using the zip file (packet) or individually from here.
Fill out the Forms and prepare the required supporting documents to submit to the OBPVS.
