This License Wizard should only be used as a guideline to determine if you need a License of State Authorization.

License Wizard Step 4 - No to both conditions

Apply as a "school".

If your response was "no" to the previous conditions (when a course is more than 32 hours or offered more than 3 times/year) then apply as a school.

How do I apply for the License of State Authorization as a school?

  • Contact the OBPVS Staff for the main application and instructions.
  • Download the remainder of the Forms using the zip file (packet) or individually from here.
  • Fill out the forms and provide the required documents.
