If your response was "no" to the previous conditions (when a course is more than 32 hours or offered more than 3 times/year) then apply as a school.
How do I apply for the License of State Authorization as a school?
- Contact the OBPVS Staff for the main application and instructions.
- Download the remainder of the Forms using the zip file (packet) or individually from here.
- Fill out the forms and provide the required documents.
