This form is used to provide medical insurance information to Child Support Services about an employee.
National Medical Support Notice MED 5
This form is used by Child Support Services to inform the employer and plan administrator there is a court order requiring the enrollment of a child on a child support case and identifying the employee who is required to cover the child. The employer responds using Part A and the plan administrator responds using Part B when appropriate.
Income Withholding Order Form (Currently unavailble at this time.)
This form is used to inform the employer there is a court order requiring that wages are withheld from an employee’s check to pay for their child support order.
Oklahoma New Hire Reporting Form
This form is used to report the hiring or employment of any person hired by and employer in the state as required by Section 2-802 of the Oklahoma Employment Security Act. This information will be forwarded to Oklahoma Child Support Services to determine whether an order is in place on said employee.
