The child support computation form is the legal document used to calculate the child support obligation for a noncustodial parent (obligor) as required pursuant to Section 120 of Title 43 of the Oklahoma Statutes. The form must be signed by the judge and attached as an exhibit to orders which establish or modify a child support obligation. The Insurance Premium Worksheet is used to compute the monthly health insurance premium for the children in the case. Health insurance is reasonable in cost when the share of the premium for the parent providing the coverage does not exceed 5% of that parent’s gross monthly income.
