Provider Search
Find contracted agencies that provide services for the following Home and Community Based programs:
- ADvantage Waiver
- Living Choice
- Medically Fragile Waiver
- State Plan Personal Care (SPPC)
How to Find Contracted Providers:
Choose one of two ways to search:
Option 1: Use Filters Select your desired Program, Service Type, and County from the filter menu.
Option 2: Use the Map Click on your county in the map. Scroll down to see all contracted providers in that county.
New Applicants
Pick your Case Management and Home Care provider. Pick a backup, too.
When you meet with a nurse, they will will ask which Case Management and Home Care provider you want. The nurse will write down your choices during your medical assessment. If you qualify for the program, you will get the provider you chose, if available.
Current Members
Tell your Case Manager if you want to change providers. They will help you complete the provider agency change.
*State Plan Personal Care members: Tell your current Home Care Agency if you wish to change Home Care providers. They will notify OKDHS of your request.
For Provider Agencies:
Need to change something in your listing? Update it in the Harmony System!
From the Provider Prime Role:
- Click Providers Tab → Edit → Edit Provider
- Update your phone, fax, address, website, or case processor, as applicable
- Click File → Save and Close
Your changes will appear in the Provider Search Tool the next day.
