This bulletin is regarding the Oklahoma DHS CAP-DDS Provider Cost and Wage Survey Template (“Survey”). Survey documents (Survey Templates and Instructions) may be accessed by clicking on the “Library Site (Provider Staff)" link on the Harmony home page under the heading “Tasks.”  Step by step instructions to access the documents begin on page two (2) of this bulletin.  If you have any problems accessing the Survey Template, please email SUPP-ASD-HELPDESK@OKDHS.ORG for assistance.

A few things to note about the Survey documents:

• Survey File Format: Please download and save the Survey template as ‘.XLSM’. By default, the template downloaded from the website should be in the ‘.XLSM’ format. If you save the file in another format, you will lose specific functionality within the workbook.
• Oklahoma DHS CAP-DDS Provider Cost and Wage Survey Instruction Manual: The instruction manual provides detailed instructions to assist in completing the Survey.
• Survey Submission: Please submit the completed Survey workbook to OKDHS-Rates@guidehouse.com and calvisi@guidehouse.com by Friday, September 30, 2022.

Links to the recorded trainings will be made available to all organizations shortly after the webinars end. Guidehouse will share a Frequency Asked Questions (FAQs) document after the session to address questions from all stakeholders and assist the survey respondent. Survey documents (Survey Templates and Instructions) may be accessed by clicking on the “Library Site (Provider Staff)" link on the Harmony home page under the heading
“Tasks.”

When the page opens, click on the folder labeled Medicaid Services Unit,

Then click on the folder labeled Cost & Wage Survey Documents, the first folder on the page.

Inside this folder are two subfolders, one for All Providers (non-HDM) and one for Home Delivered Meal Providers.  Click on the appropriate folder for your service type and the Survey Template and Instruction Manual are available for download.

Remember to save the Survey template as an ‘.XLSM’ file.
