Memorandum of Understanding for Access to the Harmony System
Please find attached the Memorandum of Understanding (MOU) between the Providers for ADvantage and State Plan Personal Care Services and the Department of Human Services.
The purpose of this document is to establish an agreement between DHS and the Provider agency for the purpose of operating in the Harmony Waiver Information Management System in order to conduct business for the ADvantage Waiver and State Plan Personal Care Programs.
This agreement requires:
- The Provider to insert their name on the appropriate pages, and
- The agreement to be signed by the Authorized Signature Person designated on the Provider Certification Application on file with the MSU-AA Contracts department.
If you have staff attending the in-person training in June, please complete this agreement and fax to the MSU(405-230-8036) no later than 6/1/2018 Otherwise the MOU will be needed by 6/30/2018. Individual user access will not be set up until the MOU is received from the agency.
Please do not include a cover sheet as the fax will be received in an internal mailbox.
Starting the first part of June, logins to the Harmony test site will be issued to any agency that has completed the MOU.
