The Title V Maternal and Child Health (MCH) Block Grant provides funding to ensure vital services for women, infants and children, including children with special health care needs. In our state, the Oklahoma Department of Human Services (DHS), which administers Title V services for children with special health care needs, collaborates with the Oklahoma State Department of Health (OSDH), which administers Title V services for women and children.

Every five years, Oklahoma’s Title V Program seeks feedback from key partners, stakeholders and families to help develop a State Plan for Services. To help us determine priorities for the next five years, we ask that you please complete a short, 10-question survey. Please feel free to share the survey link with others who may wish to participate. With your feedback, we can improve the health of Oklahoma’s women and children.
