OKDHS:2-11-91. Voluntary payroll deductions

(a) For voluntary payroll deductions, such as supplemental insurance premiums, charitable contributions, credit union deductions, or employee association membership dues, the Oklahoma Human Services (OKDHS) employee contacts the company and the company sends the appropriate documents to the Financial Services Payroll Unit for processing.

(b) The employee completes Form 10PL022E, Voluntary Deductions Cancellation, to cancel any voluntary payroll deduction.  Changes other than cancellation must be addressed directly with the company or credit union.
