OKDHS:2-11-92. Changes in withholding tax

     When new employees enter duty or when employees change federal or state withholdings, federal form W-4, Employees Withholding Allowance Certificate and Oklahoma Tax Commission Form OK-W-4, Employee’s State Withholding Allowance Certificate is completed.  Completed and signed forms are sent to the Financial Services Payroll Unit by email to STO.Finance.Payroll.CTK@okdhs.org.
