1.Specialized Community Home (SCH) assessment process.The Residential and Placement Services Program Unit sends a pre-resource to the district where the prospective SCH is located.
(1) The resource specialist:
(A) assesses the home in the same manner a traditional foster home is assessed per OAC 340:75-7-18;
(B) completes each required background check of each SCH applicant and adult household member;
(C) completes Form 04AF003E, Resource Family Assessment – Bridge Family Profile, or refers the Bridge resource family assessment to an OKDHS contractor for completion;
(D) schedules training for each applicable adult residing in the prospective SCH;
(E) reviews all information gathered during the assessment and submits the assessment to the resource supervisor for approval, denial, or postponement;
(F) maintains a resource paper file; and
(F) completes a re-assessment annually, per OAC 340:75-7-94, for the approved SCH.
(2) The resource supervisor:
(A) approves or denies the resource family assessment, per OAC 340:75-7-18.
(B) notifies the Residential and Placement Services Program Unit of the resource family assessment disposition.
(C) notifies the Residential and Placement Services Program Unit of concerns regarding the SCH;
(D) approves or denies the re-assessment per OAC 340:75-7-94; and
(E) notifies the Residential and Placement Services Program Unit of the disposition re-assessment disposition.
(3) The Residential and Placement Services Program Unit:
(A) creates a resource when the SCH is approved as a foster home; and
(B) assigns the resource specialist a secondary assignment of the SCH.
