Oklahoma Rural Health Transformation Program Announces Public Grant Application

OKLAHOMA CITY – The Oklahoma Rural Health Transformation (RHT) Program has released its first public grant application through a Notice of Funding Opportunity (NOFO) for the Community-Led Wellness Hubs: Microgrants program. Applications are due by 11:59 p.m. CT on April 13, 2026.

The Microgrants program is one of 29 programs included in Oklahoma’s RHT Program initiatives.

Microgrant funding will support one-time purchases of equipment and other lasting assets that can help improve health care access, health outcomes and overall community wellness in rural Oklahoma communities.

Eligible organizations must operate in a rural county or maintain a branch serving a rural county, serve communities with populations of 55,000 or fewer residents and maintain required federal registration and insurance documentation.

“This microgrant program is a direct result of what we heard during our listening sessions in our program planning phase,” said Jackie Kanak, Director of Operations and Population Health for the Oklahoma RHT Program. “Communities told us that even small investments, like diagnostic equipment or wellness resources, could make a meaningful difference in access to care and overall health.”

Program staff will host a webinar from 1-2 p.m., Thursday, March 19 to provide an overview of the Microgrant application and answer questions. A recording will be posted to the application website following the meeting.

The Microgrant NOFO and webinar link are available on the RHT Program website at Oklahoma.gov/health/RHTPfunding.

The Oklahoma RHT Program will also host several public engagement meetings in April for residents interested in program updates and upcoming funding opportunities. Details for these events have been posted on the program website.

The Oklahoma RHT Program is part of a historic $50 billion federal investment by the Centers for Medicare & Medicaid Services (CMS) authorized by the One Big Beautiful Bill Act. Oklahoma secured $223,476,948.62 million for the first budget period of a five-year grant that focuses on helping people get the care they need where they live, leading to a healthier, thriving state.
