Service Oklahoma can receive the Officer's Affidavit electronically and via U.S. Mail. Service Oklahoma will accept electronically submitted, legible, affidavits in PDF format that are sent to sokduiaffidavits@service.ok.gov.  Each affidavit must be attached to a separate email.
