How do I Report a Change?
To report a change for your benefits, use the OKDHS Report a Change at https://changerequest.dhs.ok.gov/
Go to Report a Change, select the card for “Change in Household Circumstances Form”, and follow the prompts. Reporting a change is easy and takes less than five minutes.
