Dear Daily Living Supports and Agency Companion Providers,

RE: THERAPEUTIC LEAVE UNITS

The DDSD Provider Support Center has been authorized to process POC changes for DDSD Waiver Daily Living Supports (DLS), Agency Companion Services (ACS) and therapeutic leave units effective July 1st, 2010. This does not include the original request for DLS or ACS but when policy permits the support center will be able to increase or decrease the therapeutic leave units and the corresponding unit of DLS or ACS. 

By allowing providers to initiate these changes with the Support Center, we will be able to expedite the processing of such requests and enable providers to submit claims in a timely manner.  

Please have the following information ready when you call or email the DDSD Provider Support Center for such requests.

• Consumer name and ID number
• Provider ID number and Location
• Date span being requested
• Authorization number
