Amended Executive Order 2025-16 Attestation FAQs
Dear DDS Providers,
Gov. Kevin Stitt issued Amended Executive Order 2025-16 on Jan. 15, 2026. The amended executive order (EO) requires all OHCA-contracted providers to submit a signed attestation affirming compliance with Oklahoma State Law and OHCA’s provider agreement
How do I complete the attestation?
1. Complete and sign the attestation form. The signature must be either a handwritten (wet) signature or a verified digital signature (e.g., Adobe Sign, DocuSign). Typed names or plain text signatures will not be accepted.
2. Upload the completed and signed form on the Attestation Submission page by 5 p.m., May 15, 2026.

What if a provider does not complete the attestation?
After May 15, 2026, there will be no mass termination of Medicaid contracts. However, contracts will be reviewed individually. If an attestation has not been received, OHCA will take action; however, no details have been provided regarding the specific disenrollment process or the expected timeline.
