Dear Caregiver,
You are receiving this letter because you previously received a set of respite vouchers in the amount of $300.00 on behalf of your family member whose name appears on the Waiver Request List for Developmental Disabilities Services (DDS) Home and Community Based Services. We are excited to inform you that DDS has received permission to provide you with one more set of vouchers in the amount of $230.00. This is a one-time benefit that will not be repeated.
Requirements for this Respite Voucher Program are:
- Caregiver and care recipient must reside in the same home;
- Care recipient is on the Waiver Request List;
- Care recipient is 18 years or younger; and
- Caregiver does not receive aid from the regular DDS Respite Voucher Program.
Use of these respite vouchers does not affect your status on the Waiver Request List or the Family Support Assistance Payment program.
Please be reminded the vouchers must be used within 90 days of the date issued and can only be submitted after the respite has been provided. If you have any questions about these additional set of vouchers please contact Vickie Thomas at (405) 521-6521 or by email at DDS.RespiteVoucher@okdhs.org.
