Dear Caregiver,
You are receiving this letter because your family member’s name appears on the Waiver Request List for Developmental Disabilities Services (DDS) Home and Community Based Services.We want to inform you of a one-time benefit available only to individuals on the DDS Waiver Request List.
The DDS Respite Voucher Program provides vouchers to a caregiver of an individual diagnosed with a developmental disability to help pay for respite, allowing the caregiver to take a break from caregiving duties.DDS has been provided an opportunity to waive some requirements for this program in order to provide this one-time benefit.
Requirements for this Respite Voucher Program are:
- Caregiver and care recipient must reside in the same home
- Care recipient is on the Waiver Request List
- Care recipient is 18 years or younger
- Caregiver does not receive aid from the DDS Respite Voucher Programs.
(DDS received special permission to authorize this one-time respite voucher for individuals who are under the age of 19 and/or receive the Family Support Assistance Payment, i.e. $250.00)
If you would like to take advantage of this opportunity, please complete and submit the application enclosed with this letter to the address, fax or email address listed below. Approval for this program does not affect your status on the Waiver Request List.
