Dear Provider:

The electronic provider incident reporting system will be unavailable the weekend of September 11 through 13, 2020.To ensure timely submittal, all known incidents must be submitted prior to 5:30 p.m. on September 11. For incidents that occur between

September 11 through 13, you may resume electronic reporting the next available business day, Monday, September 14, 2020.

During the current pandemic period, reporting requirements for critical incidents must occur within three (3) business days of observing or discovering the incident.If staff is not present and the incident is discovered, use the 'date discovered' as the 'date of the incident'.

If at any time technical difficulties prevent you from submitting an incident report within required time frames, contact to resolve the issue. When you are able to submit the incident, note the reason for the delay in the Action Taken section.
