Dear Nonprofit Waiver Service Provider: 

RE: Fees for Motor Vehicle Records

As a nonprofit contractor with Developmental Disabilities Services of the Oklahoma Department of Human Services, Oklahoma Statute 47 OS Chapter 2 Article 1 Section 2-110 (C) (7) exempts you from fees for the release of any motor vehicle record required when requested in the performance of official duties.

The attached letter will serve as evidence that you are contracted with DDS as a Home and Community Based Waiver provider for fiscal year 2017.  A copy of this document can be attached to the completed DPS 303RM 0097 and sent to DPS.  

Please list 47 OS Chapter 2 Article 1 Section 2-110 (C) (7) as the statutory citation on number 7 of the DPS 303RM 0097.
