Effective November 1, 2012, the rates for services listed will increase based on the service description identified in the following table:

Please update your billing processes to reflect the rate increases for services provided on or after November 1, 2012.
A contract modification is enclosed for your agency approval and signature so adjustments can be made for reimbursement. Please return with authorized signatures at your earliest convenience.
