Update DDS Waiting List Contact Information:

Dear Waiver Applicant:

This is your opportunity to provide current contact information since your name is on the request list, also known as the waiting list, for Developmental Disabilities Services (DDS) Home and Community Based Waiver services. This is not for new applicants. Click here to apply for services  To ensure the request list has correct information, we are asking you to provide your contact information yearly. 

Please enter applicant name, address/contact information, date of birth, and alternate contact person as requested below.   Also, it is important to let us know if you want to stay on the request list for Waiver services.

Ways to provide your information:

3. Phone– Call DDS with your contact information, 405-500-1866,
   Monday through Friday, 8:00 a.m. to 5:00 p.m., except state holidays 8:30 to 4:30

It is important for you or your contact person, such as a family member, advocate, or guardian to provide this information anytime there is a change.  If DDS is unable to reach you or your contact person with the information provided, this may result in a delay of service and, ultimately, closure of your application for Waiver services.
