This School Food Authority (SFA) operates the Special Milk Program.

The following areas were Findings Identified During the Administrative Review (AR) for the Operation of the Special Milk Program (SMP):

Counting and Claiming: A review of the counting system demonstrated systemic counting and claiming errors for the review period, December 2024, which results in a performance standard 1 violation [per federal regulation 7 CFR Part 210.10 (b) (i)].

Below are the differences the OKDHS School Nutrition Programs (SNP) reviewer identified, in Lakewood Christian School’s (LCS) Special Milk Program’s claim for December 2025.

There was an under claim of 237 milks.

Federal regulation [7 CFR Part 215.10 (d)] requires School Food Authorities to check the claim to “…certify the claim is trye and correct.”

To demonstrate compliance with counting and claiming requirements:

·       Review your counting and claiming system to ensure accuracy daily meal counts are taken and recorded.

·       Provide copies of the: milk rosters with edit check for February 2025.

·       Provide the above copies, along with LCS’ plan for corrective action to demonstrate compliance and understanding.

This School Food Authority (SFA) operates the Special Milk Program.

The following areas were corrected during the Procurement Review (PR):

·       Code of Conduct: At the time the PR was commenced, the code of conduct provided had the name of a former staff member listed as responsible for the interpretation of the code [federal regulations requiring the code of conduct 2 CFR Part 200.318 (c) (1), 2 CFR Part 400.2 (b) (1), and 7 CFR Part 215.14].  Prior to the completion of the on-site portion of the Administrative Review (AR), Lakewood Christian School (LCS) took corrective action providing a code of conduct compliant with federal requirements.

No further action is required for the above.

There were no further findings for the procurement review of your school.
