Provider Survey Postcard
You are invited to participate in the 2018 Child Care Services (CCS) Provider Survey. This survey is conducted periodically to learn more about the availability and effectiveness of services and programs. The survey results will be incorporated in the 2019 State Child Care Plan.The survey should take approximately 10-15 minutes to complete. Participation is voluntary and no information about you or your program is requested. Your responses will remain anonymous and will be analyzed by the Office of Performance Outcomes and Accountability (OPOA); only group data will be reported.

You may access the Provider Survey online or scanning the QR code in the top right corner of this postcard. Paper copies of the survey may be requested via the contact information below. The survey will be available until 6:00 p.m. on April 23, 2018.
