Subject: Harmony HCBS Community Integration Forms Training

Date: December 14, 2020

By March 2023, all Adult Day Health (ADH) and Assisted Living (AL) providers must be in 100% compliance with the Centers for Medicare and Medicaid Services (CMS) rules for Home and Community Based (HCB) Settings. ADvantage is offering training to ADH and AL providers on HCB Settings requirements. The training will highlight the CMS rules and timelines for HCB Settings compliance, as well as introduce providers to Harmony Activity Assessment and Community Integration forms. ADvantage is conducting this training to answer questions, offer guidance, and provide tools that will assist in reaching compliance with these rules.

ADvantage ADH and AL staff responsible for Quality Assurance/Improvement and HCB Settings compliance should attend this training. This training is mandatory for facility Activity Directors and Activity personnel. The new Harmony HCBS forms are Member specific forms and will be required for ADH providers to utilize for reporting beginning January 1, 2021.

Training will be held on Monday, December 28, 2020, from 10:00 am to 12:00 pm via Zoom meeting. Please pre-register by close of business on Tuesday, December 22, 2020, using the following link:

After completing the online registration, please check your email for information about joining the Zoom meeting. You will need to join from a computer with stable internet connection. Having access to computer audio with microphone and video camera is encouraged. You can join the audio portion from a telephone if necessary by simply following the prompts on the screen.
