Expired Oklahoma Health Care Authority Contracts

The following Provider Contracts to provide ADvantage and SPPC services expired on November 30, 2018.

Home Care
Case Management
Nursing Facility
Adult Day Health
Environmental Modification
Assisted Living
Most Hospice

If you have NOT renewed your contract through Oklahoma Health Care Authority (OHCA), please go on line and fill out your new contract immediately.

Claims are already being denied by OHCA, and will continue to be denied until contracts have been renewed.
