Subject:  Survey of Members’ Needs for COVID-19 Vaccinations, and Dentures, Glasses and Hearing-Aid Assistance

Date:  June 30, 2021

The ADvantage waiver program is seeking the assistance of all Case Managers in identifying Members with vaccination needs as well as assessing the extent of potential need for dentures, glasses and/or hearing-aid assistance.

To identify and address the needs of vulnerable adults who may still need/want a COVID-19 vaccination, we are requesting that each Case Manager reach out to all Members on their caseloads to ask the survey questions and document Member responses to questions using the link below.  The MSU also requests that, as part of this Member interview, Case Managers ask each Member if they have a need for dentures, glasses, and/or hearing-aids to estimate needs in these areas.

Case Managers should complete the survey for every Member on their case load.  Upon completion of the survey by the Case Manager, results will automatically submit to the MSU.  The survey will be active from 6/30/2021 through 7/31/2021.

To support Case Managers in taking on this additional work, the MSU is approving an additional 25 case management units per Member for all plans that are active during the timeframe of 6/30 through 7/31/2021, the date by which the surveys are due.  A similar process will be put in place for all Members new to the program on 8/1/2021 or after.

To avoid Case Managers having to submit Service Plan Changes for the additional 25 units for every waiver Member, the MSU is seeking to have these additional units added in bulk by the WellSky vendor.

Please click here to link to the vaccination survey, and save the site in your Favorites to access going forward.  If completing multiple surveys one after another, you will be offered the option to “Submit Another Response” after each survey submission.  The survey may be completed on any device, desktop, laptop, or mobile device.

Thank you for assisting waiver Members to receive the support they need to be fully vaccinated against the Coronavirus.
