Subject:  Resuming Wet Signatures

In anticipation of the approaching end of the public health emergency (PHE) and consistent with resuming in-home visits, the Medicaid Services Unit is resuming obtaining wet signatures for Members’ initial and reassessment service plans and certain plan changes for ADvantage and State Plan Personal Care when at all possible.

Wet signature rules are the same as pre-PHE requirements:

  • New and reassessment service plan submissions, and
  • Plan changes that reduce and/or end a Member’s services.


The new signature form is constructed to allow for verbal consent whenever necessary as well as to accept wet signatures as appropriate.  The flexibility for verbal consent remains in effect during the time the state is under Appendix K, which extends up to six months after the end of the public health emergency, however we encourage Case Managers to resume obtaining wet signatures when at all possible.  The new form will be put into use effective 5/16/2022.
