Subject:Management of Medications in Harmony

In order to help facilitate consistency in the use of specific tabs in the Harmony system, the attached document has been created to guide in management of medications in the system.

This process will ensure the following:

  • The Medications Tab is always kept current
  • All current medications are listed as appropriate on the Nurse Evaluation and UCAT III
  • All reported medication changes since the last visit are listed on the Monitoring/Progress Note
  • Duplication of meds on the Medications Tab does not occur

Document changes:

Monitoring/Progress Note

When the question of "Recent changes to medications" is answered "Yes" the "Medication Changes" question as shown below will display. This is an area to list new or changed meds that will map back to the Medications Tab.

Nurse Evaluation

The question shown below has been changed to read "Medications". All current medications at the time of the visit should be listed on this document. User will utilize the "search" or "Attach" feature to load medications already listed on the Medications Tab in Harmony.

UCAT III

A new question has been added to show that the CM did review the medications on the Reassessment Home Visit. All current medications at the time of the visit should be listed on this document. User will utilize the "search" or "Attach" feature to load medications already listed on the Medications Tab in Harmony after the Home Care RN finishes the Nurse Evaluation which should update the Medications Tab.

Other Medication:

If a Medication is not available in the "Search by Medication" field, "other" can be selected and the actual name of the medication listed below in the Medication Info field.
