AAU Provider Update: ADvantage Members Living in an Assisted Living

ADvantage Members Living in an Assisted Living – Provider Communication form (ADv9) Notification Regarding Member’s Hospitalizations and Other Stays from the ALC

While residing in the Assisted Living, it may become necessary for the Member to receive skilled nursing services or hospitalization due to illnesses or injuries that are outside the scope of what services the Assisted Living can provide.

If an ADvantage Member leaves your facility for any stay at a Hospital or Nursing Facility, or returns to your facility from any stay at a Hospital or Nursing Facility, please submit Form 02CB009E (.doc, 1 pp, 245 KB), Provider Communication (ADv9), to the AAU via ASD.PROVIDERQUESTIONS@OKDHS.ORG, noting the date that the Member left the ALC to be admitted to these institutions or left these institutions to be re-admitted to your facility, as well as any other vital information.  It is suggested that you also forward these emails to the ADvantage Case Managers for their records.  This form should be submitted within 24 hours of the Member exiting the ALC to be admitted to one of these facilities or re-entering the ALC from one of these facilities.
