To: All ADvantage Case Management Providers with Members residing in Village at Oakwood Assisted Living
Effective today, June 4, 2012, all requests for re-tiering of Village at Oakwood Assisted Living Members has been suspended until after July 2, 2012.
Please hold all request forms and documentation for re-tiering of a Village at Oakwood Assisted Living Member until after July 2, 2012.
