​Business Name Change to Kindred at Home

Businesses impacted:

Girling Health Care – OKC and Tulsa

Gentiva Health Services – Hugo and Lawton

Effective February 6, 2017, the name for the above listed agencies will be changed to Kindred At Home.  The address, telephone, and fax numbers will remain the same. This is just a name change.

All service plans or addendums for Case Management and Home Care submitted after February 6, 2017 must have Kindred At Home (branch name) listed in order for them to be processed.  Plans submitted and dated before February 6, 2017 will be processed as usual.
