Provider Update: June 24, 2016

​Delta Nutrition Home Delivered Meal Extension

Please be advised that ASCOG Area Agency on Aging (AAA) has extended their contract with Delta Nutrition home delivered meal provider to September 30, 2016.  ASCOG Area Agency on Aging anticipates publishing a request for proposal for their nutrition program in July 2016. Due to this extension, the Medicaid Services Unit has also extended the Delta Nutrition contract pending the AAA's final contract award results.

Action may be necessary, please review the following options:

• If your Member has already selected another home meal provider, and an addendum has been submitted with that change, and the Member wishes to remain with the newly selected provider, no action will be necessary.

• If your Member has already selected another home meal provider, and an addendum has been submitted with that change, and the Member wishes to return to Delta Nutrition for an additional three months, please submit an addendum to facilitate that change.

• If your Member has not selected another home meal provider, via addendum, the Member may remain with Delta Nutrition until September 30, 2016 or may change to another meal provider at any time during these three months.

Please note the Member always has the choice to add Personal Care hours in lieu of a Home Delivered Meals agency.
